Salary: £28,353 - £31,756 per annum + 6%

Hours: 36 hours per week

Based at: Haringey Multi Agency Care & Coordination (MACC) Team

Contract: One year (possible extension)

The Bridge Renewal Trust, in partnership with the NHS, are delivering care navigation services for frail patients in Haringey.

As a Care Navigator, you will work as part of a multidisciplinary team to support frail patients in Haringey by providing individual holistic assessments and directing patients to appropriate services, including activities that may help to promote their health, wellbeing and independence.

Are you a passionate and dynamic individual with excellent organisational and interpersonal skills; can you work in a multi-disciplinary team, working closely with other healthcare professionals, the general public and voluntary sector?

We are currently recruiting a Care Navigator who will focus on helping patients to make full use of community and voluntary sector services available locally, as well as those provided by health and social care agencies. The key requirements for this post are experience of working with clients providing person-centred care and being familiar with voluntary and community services to respond holistically to patients' health and wellbeing needs.
