For over 135 years we have brought people together through volunteering, community activity and neighbourly support. Today we work with older adults, families and local residents to reduce loneliness, improve wellbeing and strengthen community connections.
We are seeking a Neighbourhood Health Coordinator to help strengthen the relationship between local communities and health services whilst leading an exciting new community outreach programme focused on healthy ageing and frailty prevention.
Purpose of the Role
The Neighbourhood Health Coordinator will play a key role in connecting older residents with local health, wellbeing and community support.
Working across Time & Talents, local Integrated Neighbourhood Teams (INTs), social prescribing services and community partners, the postholder will coordinate a programme of neighbourhood outreach activity designed to improve health outcomes, reduce loneliness and support older people to age well.
The role will combine programme coordination, partnership development, community engagement and direct delivery.
The postholder will spend time embedded within local health partnerships, helping to strengthen collaboration between statutory and voluntary sector organisations and ensuring local residents can access the support they need.
Key Responsibilities
Community Health Outreach
Coordinate and support delivery of a programme of community outreach events across Southwark.
Work alongside community organisations to engage older residents who may be isolated, vulnerable or at risk of frailty.
Support residents to access local services, activities and community assets.
Promote prevention, wellbeing and healthy ageing messages.
Partnership Development
Build strong relationships with:
o Integrated Neighbourhood Teams
o Social Prescribing teams
o Primary Care Networks
o Community health services
o VCSE organisations
o Housing and faith partners
Act as a key point of contact between Time & Talents and local health partners.
Identify opportunities for collaboration and service development.
Community Engagement
Engage with residents in community settings including sheltered housing, community centres, faith venues and neighbourhood hubs.
Ensure outreach activities are accessible, inclusive and responsive to community need.
Gather resident feedback and insight to influence future service delivery.
Programme Coordination
Coordinate delivery schedules, partner engagement and event logistics.
Monitor project activity and outcomes.
Collect and report data in line with project requirements.
Produce case studies and examples of impact.
Integration and Learning
Attend relevant neighbourhood health meetings and forums.
Share learning between partners and contribute to service improvement.
Support the development of sustainable pathways between health and community services.
General
Work in accordance with Time & Talents policies and procedures.
Promote safeguarding, equality, diversity and inclusion.
Undertake other duties appropriate to the role.
